• This index was developed in 2739 patients admitted to hospitalist and nonhospitalist physicians at the University of Chicago Hospital from 1997 -1999 (mean age 78 years, 63% female, 26% 1-year mortality).
• The index was internally validated in 3643 patients from the same sample (mean age 78 years, 63% female, 26% 1-year mortality).
• Discrimination: This risk calculator sorts patients who died from patients who lived correctly 65% of the time (c-statistic).
  
• Calibration: The model was well calibrated at all risk levels, with less than 10% difference between estimated and actual mortality rates.
