In the original study, the following items were based on International Classification of Diseases, 9th Revision.

1. Please indicate if your patient has:
• lymphoma or leukemia?
 
• acute renal failure?
 
• metastatic cancer?
 
• localized cancer?
 
• stroke?
 
• congestive heart failure?
 
• chronic lung disease?
 
• chronic renal failure?
 
• diabetes with end-organ damage?
 
• pneumonia?
 
2. Does patient have an admission albumin level less than or equal to 3.5?
 
3. Does your patient have an admission creatinine level greater than 1.5?
 
4. Does your patient have dementia (Alzheimer disease, organic brain syndrome, or chronic cognitive impairment before admission)?
 
5. Does your patient’s medical record indicate that they need help from another person to walk or are unable to walk?
